Policy Statement

Payroll Services will process payroll deductions imposed by State and Federal law, and uphold participation in direct deposit of salaries, required by new employees of the University.

Prior to initial payment to employee, Payroll Services must receive completed originals of tax forms W-4, NC-4 and direct deposit authorization with regards to mandatory deductions.

I. Salary

All employees are required to complete and submit the following forms regarding withholdings and eligibility:

Mandatory Federal and State Tax Forms

Federal Tax Form W-4

North Carolina Tax Form NC-4

Employment Eligibility Verification Form I-9 (collected by Human Resources at the initial interview or within three business day from the start date of employment) Social Security Number assigned by the Social Security Administration

Mandatory University Forms

Direct Deposit Enrollment Form

II. Benefits

The University requires each new employee to participate in a New Employee Benefits Orientation through the Benefit Programs Administration in Human Resources. Once selected and appropriately authorized, benefit deduction information for each employee will be transmitted to Payroll Services through the benefits web system for processing.

Reason for Policy

The University provides this policy to apprise its employees of the necessity for Payroll Services to obtain completed mandated documents prior to release of pay.



Special Situations

When assignment of a Social Security number is pending, an official statement that the official application has been submitted will be temporarily accepted only until the employee has received his/her official Social Security Card.
